Instructions:
Please fill out this form before sending equipment in for maintenance or repair.  When you click "Submit," this information will be sent to TimeMark and the contact e-mail address.

After submitting the form, you may pack and ship your equipment.  No other paperwork is necessary.  Our shipping address will be shown on the next screen or you can find it on our Contact Information page.

Repairs are handled in the order they are received.  Units are usually evaluated within 1 - 4 weeks after they arrive at TimeMark.  Your wait time will depend on how many other counters are also waiting for repairs.

Your equipment will be evaluated based on the information you give us below.  If all the work is covered by warranty, we will simply fix the units after they are evaluated and then send them back.  Otherwise, we will give you a cost estimate and wait for your approval.
